Peyronie's disease is a condition which affects the penis, usually resulting in painful erections, scarring in the penis and a bend in the erection. It is caused by damage to the penile tissue creating a scarring under the skin, known as a plaque. Depending upon the size and location of the plaque, it can cause the penis to bend upwards, downwards or become indented.

The condition is fairly common, affecting approximately three per cent of men. It can occur at any age, but most typically affects men in their forties and fifties. There is a link between Peyronie's disease and the hand condition Duypetren's contracture, which causes one or more fingers to bend into the palm. About one in ten men with Duypetren's contracture have Peyronie's. However no-one knows exactly why some men get Peyronie's disease and others don't; it appears to be a matter of chance.

The first symptom of Peyronie's disease is usually painful erections, or a bend in the erection, or both. Sometimes, men notice lumps in the shaft of the penis, although they may not be very obvious. Symptoms may come on suddenly or more gradually. This is known as the acute inflammatory stage of the disease.

The length of this first phase varies, but usually the disease stops progressing after six to 18 months. The second phase is called the acute stage. The penis stops bending and erections may cease to be painful. The lumps in the penis are likely to become harder and more obvious.

Men are left with a degree of deformity which varies widely. For men with a minor deformity, there is no need for surgery. But many men experience sexual dysfunction, due to the degree of curvature in the penis and often accompanying psychological factors. If the penis bends to 60 degrees or more, sexual intercourse may not be possible.

Unfortunately there is no guaranteed cure for Peyronie's disease. Lots of treatments have been tried, but none of them have been proven in controlled trials to be successful. This means that for most people in the initial stages of the disease, it is simply a matter of waiting for the disease to stop progressing after about 6 to 18 months.

The only reason to consider surgery is in order to straighten the erection. Only men whose erection is so bent that they are unable to have satisfactory sexual activity should undergo surgery. However, surgery will not improve potency or remove penile lumps.

Surgery should only be performed once the disease is stable. This means that the pain has gone and that the bend has not got worse for at least three months. There are three principle operations for Peyronie's disease: the Nesbit procedure, the Lue procedure, and in the most severe cases, insertion of a penile prosthesis.

Nesbit's operation is the easiest and most straightforward operation to perform for Peyronie's disease, but it has one major drawback - it causes shortening of the erection. Shortening is approximately 1cm per 30 degree bend of the penis. However, providing men are thoroughly assessed and this drawback is discussed, it is rarely a major problem after surgery.

For the majority of men, the Nesbit procedure is the best choice. The principle of the operation is to remove some tissue from the long side of the bent penis, thereby shortening the long side of the bend and straightening the erection.

The Lue procedure does the opposite of the Nesbit's and makes the short side of the bend longer. However the operation is harder to perform and has a higher risk of complications. Shortening of the erection can still occur, the risk of impotence after surgery is greater that with Nesbit's operation and there is a risk of damaging the delicate nerves which go to the end of the penis, resulting in the glans of the penis being numb. Incision and grafting can also be used to treat deformities of the penis where Peyronie's disease causes indentation on one side. The graft is inserted into the indented area to expand the penis's circumference.

A penile prosthesis is only used when there is very severe impotence, which can't be treated by other means such as drugs. The operation has many drawbacks, but can restore satisfactory erections in selected cases. It should be considered only as a last resort.

Last night we enjoyed a perfect convergence of place with?three great wines from the?storiedBrancott Estate of Marlborough, New Zealand and the cool XVI Rooftop Lounge on a perfect New York summer night.

Our host, Jim Robertson, Global Brand Ambassador of Brancott Estate and Stoneleigh Vineyards?along with Perod Ricard showed us Brancott's Classic Range?Pinot Grigio, Pinot Noir and Sauvignon Blanc.?

There are certain precious parts of the world that are perfect places to produce wine. One?such place called?Marlborough?is a?sun drenched?yet varied temperatured fertile land on the north-eastern tip of the South Island New Zealand.

Here in 1973, before a single grape sprout had emerged Frank Yukich, then Managing Director,?said with great confidence?"Wines from here will become world famous." How right he was.

Over the years NZ Sauvignon Blanc has become world renowned. Less known is the excellent Pinot Noir and Pinot Grigio that?is made there. So Jim Robertson is on a whirlwind trip around the US to share Brancott's wines which brings us to the next place, the place of enjoyment.

Sky XVI is a hip rooftop?lounge?at 251 West 48th St. It is an indoor and outdoor?lounge designed to be reminiscent of a?Louis XVI chateau. Here we enjoyed these sun filled wines and a beautiful sunset.

Jim?told the?captivating story of how Brancott came to be the prominent wine producer in NZ's prominent wine region and?why their wines are so good.? We know wine making is?blend of nature meets science with a lot of art and?often good luck. At Brancott?nature is?certainly on their side?and they have the science down pat. It is in the art where they really achieve and are seemingly blessed with a lot of good luck. The wines we tasted are of Brancott's Classic Range?which are first quality value wines.

Of the wines we tasted,?I loved the Pinot Grigio the best. I have been drinking a lot of Pinot?Grigio latelyClassic-range-us?in an effort to newly understand this grape. I realized something last night which was that good Pinot Grigio or at least the ones I like have great texture and body. They just wrap around your tongue and fill your mouth with opulent luxury.? One Italian Pinot Grigio that delivers this is Terlano Pinot Grigio. Brancott Pinot Grigo is rich like that as well.

The Pinot Noir was very enjoyable as well. I personally like deeper Pinot Noirs, more dark plum and raisin, but Diana who usually complains that Pinot?Noir is too "thin" found this wine rich and fulfilling. I noticed that taking time to really get your nose into this wine?greatly enhanced the experience.? The nose that a wine projects is always your first sign of whether it is a good wine.

The Sauvignon Blanc which we tasted last?was right there 100% bright NZ style, yet very balanced and smooth. An extremely delicious wine that put a smile on everyone's face.

These three wines are very well priced and widely available.

IN MOROCCO their language has been made official. In Algeria they lead protests against President Abdelaziz Bouteflika?s regime. In Tunisia they are rediscovering a long-suppressed identity. In Libya they man the rebels? western front in the mountains south of the capital still held by Muammar Qaddafi. Even in Egypt?s oasis of Siwa, near Libya?s border, Berbers are finding that the revolution has given them a chance to revive their cultural rights.

?There is a Berber renaissance taking place across north Africa,? enthuses Mounir Kejji, a Moroccan Berber campaigner. In his country a new constitution, endorsed in a referendum on July 1st, officially recognises the Berber language for the first time, though parliament will decide what this means in practice; Arab nationalists and many Islamists have long demanded that Arabic be the sole language of administration and state education.

The authoritarian Arab nationalist regimes that dominated the region used to accuse the Berbers of threatening national cohesion. Now, shaken and in some cases overthrown, they have seen Berber activism take on a new lease of life. Even where they are a minority of only a few thousand, as in Egypt and Tunisia, Berbers have been able for the first time to form community associations.

Libya?s rebellion is fiercest in the Nafusa Mountains, a Berber heartland long neglected by the government. Colonel Qaddafi has refused to acknowledge Berber culture for most of his reign, describing it as ?colonialism?s poison? intended to divide the country. Only in 2006, apparently after his son Seif al-Islam intervened, did he lift a ban on the use of Berber names.

Berbers make up about 5% of Libya?s 6m-7m people, though some activists put the figure higher. In recent weeks they have set up a radio station. The rebel-controlled Libya TV, based in Qatar, now broadcasts in Tamazight, the Berber tongue, for two hours a day. In June, says Mr Kejji, a delegation of Libyan Berbers affiliated to the rebels? Transitional National Council put a linguistic query to their Moroccan counterparts: how should they write ?army?and ?national security? in Tamazight, so that Libyan uniforms could have a badge in their own language alongside Arabic?

A written script for the various Berber dialects was created only in the 20th century. Algeria?s Kabyles, a Berber people said to number 4m, have usually preferred the Latin alphabet, whereas a Tuareg alphabet, called Tifinagh, is now officially used in Morocco and has been adopted by Libyan Berbers who were banned from using it under the colonel. (The Tuareg are nomadic Berber pastoralists living mainly in southern Algeria, eastern Mali and western Niger.)

The Berber revival has rekindled enthusiasm for pan-Berber solidarity. ?There?s an awareness among Berbers across north Africa of that element of their identity which they share,? says Hugh Roberts, an expert on the Maghreb. But each country in the region, he says, has its own particularities. The dream of creating a community of 20m-plus people (estimates of the total vary widely), stretching from Egypt?s western desert to the Atlantic, would be stymied by the multiplicity of Berber dialects and by the variety of political circumstances. ?A single Berber identity exists only virtually?on the internet and among diaspora intellectuals,? says Mr Roberts.

Statue of Liberty to close for yearlong repairs (AP)

NEW YORK ? The Statue of Liberty will close for a year at the end of October as it undergoes a $27.25 million renovation that will make the interior safer and more accessible, U.S. Secretary of the Interior Ken Salazar announced Wednesday.

The renovations are limited to the monument, and Liberty Island will remain open and the statue itself will be mostly unobstructed from view, officials said in a statement.

Tegan Firth, a spokeswoman for Statue Cruises, which on an average Saturday during the summer ferries about 18,000 people to Liberty Island, said the renovation won't have a significant impact on visitation.

"The entire experience of visiting these national landmarks of the United States remains absolutely the same," she said.

The National Park Service, which manages the Statue of Liberty National Monument and Ellis Island, said it will close the monument on Oct. 28, after the 125th anniversary of its dedication.

It will be closed the following day, and workers from Joseph A. Natoli Construction Corp., of Pine Brook, N.J., will install "code-compliant" stairways and upgrade electrical and fire suppression systems, elevators and bathrooms.

The National Parks Service told The Associated Press in August 2010 that it would close the statue's crown to upgrade the stairwells and improve safety at Lady Liberty.

One reason, the service said at the time, was that the newest fire codes mandate escape routes that would allow the statue to be evacuated within two hours, but the current staircases on either side of the pedestal do not meet the standards.

The statue was closed after the Sept. 11 terror attacks for security precautions, but the base reopened in 2004 after a $20 million security upgrade. The observation deck at the top of the crown was reopened on July 4, 2009.

The National Parks Service controls the number of visitors to the crown, saying about 240 people visit each day. About 3.5 million people visit the monument every year.

The statue, a gift from France, was built in the 1880s.
